At Share the Care Hospice Inc, the patient’s primary care physician plays an important role in recognizing when hospice care is appropriate and initiating the referral for hospice services. We encourage the primary care physician to remain actively involved as part of the patient’s care team and to participate in developing and guiding the hospice plan of care.
