Social Worker

At Share the Care Hospice Inc, our hospice social workers play a vital role in supporting both patients and families throughout the hospice journey. They conduct initial and ongoing psychosocial assessments and develop a personalized plan of care that addresses emotional, social, and practical needs. Typically, the social worker meets with the patient and family one to two times per month to provide guidance, emotional support, and reassurance. Additional visits can be arranged at any time upon request by the patient, family members, or the interdisciplinary care team.

Our social workers offer assistance with sensitive and important matters such as understanding and completing Do Not Resuscitate (DNR) orders, identifying financial assistance or community resources, and coordinating nursing home placement or transfer to an inpatient care facility when needed. During times of emotional distress or crisis, they also provide compassionate counseling to help patients and families cope, ensuring no one feels alone during this journey.
