At Share the Care Hospice Inc, the Registered Nurse (RN) Case Manager plays a central role in coordinating and overseeing your loved one’s care. Through comprehensive initial and ongoing nursing assessments, the RN works closely with the primary care physician and the Hospice Medical Director to develop and manage an individualized plan of care.

The RN Case Manager typically visits weekly, or more often as needed, to monitor symptoms, manage comfort, and ensure that pain or other distressing symptoms are effectively controlled. In addition, the RN supervises care provided by licensed practical nurses and home health aides, and collaborates with the full hospice care team to address the physical, emotional, psychosocial, and spiritual needs of both the patient and family.
